Awareness of normal anatomy and recognising changes can be a tricky business as the complexity of the vulval map can be confusing. Often the term vulva provokes uncertainty, and the word vagina is commonly used as a general overarching name for female genitalia.

We are of course all different- our shapes and body parts are similar but innately unique. For most of us it is not routine to make comparisons or describe this part of our body, so it is not surprising that the vulva can be unfamiliar territory.

With this in mind:

WHAT IS THE VULVA AND WHAT IS IT’S PURPOSE ?

The word vulva aptly means covering (Latin) and refers to the female’s external genitalia, for most this will include:

• The mons pubis &#; the mound of fatty tissue that covers and protects the pubic bone.

• The Labia majora &#; commonly known as the outer lips. In some of us the labia meet , covering the inner lips and other parts, but for others these parts can be seen, as the labia majora remains slightly parted.

• The labia minora- the inner lips are located inside the labia majora and the skin is usually darker in colour , though this can vary from person to person, as can the size , shape and length of each side , often there is no symmetry, and it is just a variation of normal to have longer inner lips than outer. The labia minora is home to tiny sebaceous glands that secrete oils for lubrication.
• The clitoris is usually located between the inner folds of the vulva (labia minora), at the top. Under the clitoral hood is the head of the clitoris known as the glans which varies in size from person to person. It is estimated that there are at least s sensory nerve endings here and it is the central hub of sexual response for most females.

• The urethral opening is located above the vagina and below the clitoris, and is where were urine is excreted through a tube connected to the bladder.

• The vestibule refers to the area of skin directly around the vaginal opening and the urethra, outside the body, but inside the labia minora.

• The vaginal opening is situated below the urethra and is like a muscular tube, that connects with the cervix (neck of womb) to the external vulva. The size and shape of the vagina can vary from one individual to another.

• The perineum describes the area of skin and muscle between the vagina and anus.

Self examination of the vulva

If you are not already examining your vulva now and again, maybe now is the time. It is always good to be aware of any skin changes for instance earlier rather than later. A comfortable way to do this is to lie down, with back and shoulders propped up with pillows. Hold a mirror in one hand, and use the other hand to part the labia and have a good look.

Check for any unusual lumps, warts, breaks to skin, or indeed other changes such as colour, thinning or thickening of the skin to the Labia or surrounding areas. If you have any unusual symptoms it would be wise to get this checked by a doctor or nurse.

Pain, itching, burning and general irritation of the vulva, may be signs a common fungal infection such as thrush (candidia). A change in smell or discharge may  indicate a different type of infection such as bacterial vaginosis. Sexually transmitted infections such as genital warts can cause warty lesions to appear anywhere around the vulva. HPV ( Herpes virus) can cause painful blisters or ulcers/sores which will go away but can reoccur. Again in these circumstances it would be wise to be seen by your doctor/gynaecologist or local Sexual Health Service who will offer testing for other infections if appropriate and sign post you to other services such as dermatology or a specialist vulva clinic if necessary.

Other vulval diseases include inflammatory skin conditions such as excema / dermatistis , Lichen sclerosis, Lichen simplex, Lichen planus. Pain syndromes such as vulvodynia  vestibulitis and vaginismus,

Pre cancerous skin conditions such as VIN (vulvar intraepithelial neoplasia).

Vulval cancer is a rare cancer. Around 1, cases are diagnosed in the UK each year. That is less than 1 in every cancers diagnosed in women. It is more common in older women and many cases are diagnosed in women aged 65 or over. But around 15 out of every cases (15%) are in women under the age of Early stage vulval cancers can occur in young women and they have been seen in women in their 20&#;s. But it is extremely rare to get vulval cancer at such a young age. (Cancer research UK).

Já não existe mais um único “critério ético”, tampouco existe mais um único juiz com poder para arbitrar todos os conflitos internacionais, com base na sua própria “tábua de valores”. E já não é mais possível expulsar os “novos pecadores” do “paraíso” inventado pelos europeus, como aconteceu com os lendários Adão e Eva. Como essa supremacia acabou, talvez seja possível, ou mesmo necessário, que o Ocidente aprenda a respeitar e conviver de forma pacífica com a “verdade” e com os “valores’ de outras civilizações.

Dois acontecimentos sacudiram o cenário mundial neste início de o primeiro foi o ultimatum russo, lançado em meados de dezembro de e dirigido aos EUA, à OTAN e aos países-membros da União Europeia, exigindo o recuo imediato da OTAN, na Ucrânia, e propondo uma revisão completa do “mapa militar” da Europa Central, definido pelos Estados Unidos e seus aliados da Aliança Atlântica após a vitória na Guerra Fria. O segundo foi a “declaração conjunta” da Federação Russa e da República da China, no dia 7 de fevereiro de , propondo uma “refundação” da ordem mundial estabelecida depois da Segunda Guerra Mundial e aprofundada depois da vitória dos EUA e de seus aliados na Guerra do Golfo em Os dois documentos propõem uma “revisão” do status quo internacional, mas o primeiro contém objetivos e exigências imediatas e localizadas, enquanto o segundo apresenta uma verdadeira proposta de “refundação” do sistema interestatal “inventado” pelos europeus. Ambos, no entanto, estão apontando neste momento para uma reconfiguração profunda do sistema internacional.

No caso do “ultimato russo”, a questão imediata que está em jogo é a incorporação da Ucrânia pela OTAN, mas o verdadeiro problema de fundo é a exigência russa de revisão das “perdas” que lhe foram impostas depois da dissolução da União Soviética[1]. Depois de , a Rússia perdeu 5 milhões de quilômetros quadrados e milhões de habitantes, mas agora se propõe a reduzir essas perdas expandindo sua influência no seu entorno estratégico e afastando a ameaça ao seu território, por parte da OTAN e dos Estados Unidos. Esse ultimatum era perfeitamente previsível e veio sendo anunciado há muito tempo, pelo menos desde a “Guerra da Geórgia”, em [2]. A grande novidade agora é que a proposta revisionista dos russos deverá avançar sem guerra, através de um jogo de xadrez extremamente complexo, no qual se acumulam as ameaças militares e econômicas, mas não deverá haver um enfrentamento direto, apesar da propaganda e da histeria psicológica provocada pelos anúncios sucessivos da “invasão que não houve”, sobretudo da parte dos Estados Unidos e da Inglaterra. A Rússia obteve uma vitória imediata ao conseguir colocar todos os demais atores envolvidos em torno de uma mesa para discutir os termos da sua proposta. E o mais provável é que os seus principais pleitos sejam atendidos, sem invasão nem guerra. Além disso, nas discussões evidenciaram-se a divisão entre as potências ocidentais e a falta de inciativa e liderança da parte do governo norte-americano, que se restringiu a repetir a mesma ameaça de sempre, de que imporia novas sanções econômicas aos russos caso ocorresse a invasão que foi reiteradamente negada pelos próprios russos, enquanto a iniciativa diplomática passava quase inteiramente para as mãos dos europeus. Os Estados Unidos não receberam o apoio que esperavam de seus velhos aliados do Oriente Médio (nem mesmo de Israel), da Ásia (nem mesmo da Índia), e mesmo da América Latina (nem mesmo do Brasil). E o que é pior, para os anglo-saxões, tudo indica que a Alemanha terá um papel fundamental na intermediação diplomática do conflito, o que envolveria numa reaproximação entre os alemães e os russos, com a liberação imediata do Gasoduto do Báltico que sempre teve a oposição dos norte- americanos. Afora o fato que um eventual sucesso diplomático alemão neste conflito daria à Alemanha uma centralidade geopolítica dentro da Europa que aceleraria o declínio da influência dos Estados Unidos entre seus aliados europeus. Neste sentido, um acordo diplomático “intra- europeu” seria também uma derrota para os Estados Unidos, mas ao mesmo tempo é impossível imaginar que um acordo deste tipo possa ter sucesso sem o apoio dos próprios Estados Unidos e da Otan que é na prática um “braço armado norte-americano”.

Já no caso do documento apresentado à “comunidade internacional” pela Rússia e pela China, no dia 7 de fevereiro recém passado, as reivindicações específicas e locais dos dois países são bem conhecidas e não têm maior importância neste contexto. A importância do documento vai muito além disto, porque se trata de fato de uma verdadeira “carta de princípios” proposta à apreciação de todos os povos do mundo, contendo algumas ideias e conceitos fundamentais para uma “refundação” do sistema internacional criado pelos europeus há quatro séculos. É um documento que requer leitura atenta e uma reflexão séria, sobretudo neste momento de desestruturação do “bloco ocidental” e de divisão e fragilização interna dos próprios Estados Unidos.

O primeiro aspecto que chama atenção nesse documento aparentemente insólito é sua defesa de alguns valores muito caros ao “sistema de Westfália”, como é o caso de sua defesa intransigente da soberania nacional, e do direito de cada povo decidir seu próprio destino, desde que respeitado o mesmo direito de todos os demais povos. Ao mesmo tempo, o documento defende também algumas das ideias mais destacadas do “liberal-internacionalismo” contemporâneo, como é o caso da sua defesa de uma ordem internacional baseada em leis, do seu entusiasmo pela globalização econômica e o multilateralismo, por sua defesa da “causa climática” e do desenvolvimento sustentável, e seu apoio irrestrito à cooperação internacional no campo da saúde, da infraestrutura, do desenvolvimento científico e tecnológico, do uso pacífico do espaço e do combate ao terrorismo. De um ponto de vista acadêmico e ocidental, aliás, esse “documento russo- chinês” lembra muitas vezes o idealismo internacionalista de um Woodrow Wilson, tanto quanto lembra, em outros momentos, o idealismo nacionalista de um Charles de Gaulle.

Mas a surpreendente originalidade desse documento aumenta ainda mais com sua defesa universal e irrestrita de valores como a liberdade, a igualdade, a justiça, os direitos humanos e a democracia. Sobretudo quando assume a defesa da democracia como um valor universal, e não como privilégio de algum povo em particular ou responsabilidade conjunta de toda a comunidade internacional, com o reconhecimento simultâneo de que não existe apenas uma forma de democracia, nem nenhum “povo escolhido” que possa ou deva impor aos demais algum modelo superior de democracia, como se fosse uma “verdade revelada” por Deus. E é neste ponto que se explicita a proposta verdadeiramente revolucionária desse documento: que se aceite de uma vez por todas que, pelo menos desde o final do século XX, o sistema interestatal não é mais um monopólio dos europeus e de algumas de suas ex-colônias, uma vez que ele está formado agora por várias culturas e civilizações, e que nenhuma delas é superior às demais, nem muito menos possui o monopólio da verdade e da moralidade[4]. Ou seja, esta proposta eurasiana de uma nova ordem mundial rejeita qualquer tipo de “universalismo expansivo” ou “catequético”, mas aceita ao mesmo tempo a existência de valores universais[5].

Não haveria nada de original em tudo isso se tais ideias fizessem parte de um texto acadêmico ou de uma reflexão filosófica pós-moderna, por exemplo. O que faz a diferença nesse documento não é seu multiculturalismo; é o fato de que este multiculturalismo aparece aqui como uma reivindicação e uma proposta universal apresentada e sustentada pela segunda maior potência atômica do mundo, e pela segunda maior economia de mercado do mundo. Mais ainda, que seja uma proposta sustentada por uma potência que faz parte da árvore genealógica da civilização ocidental e, ao mesmo tempo, por uma potência e uma civilização que não pertence a esta mesma matriz, nem teve jamais nenhum tipo de vocação catequética. Sim, porque a China se desfez do seu Império milenar e só se transformou num Estado nacional no início do século XX; e foi só no final do século XX que ela se integrou plenamente ao sistema interestatal, incorporando-se à economia capitalista mundial numa velocidade e um sucesso extraordinários. Desde então, o Estado nacional chinês se comporta como todos os demais Estados europeus, mas a China nunca teve nenhum tipo de religião oficial, e nunca se propôs ser um modelo econômico, político ou ético universal &#; e por isso também nunca se propôs a catequizar o resto do mundo. Pelo contrário, a China parece fazer questão de se relacionar com todos os povos do mundo independentemente de regimes políticos, religiões ou ideologias, mesmo quando seja absolutamente inflexível com relação à defesa nacional de seus valores tradicionais e interesses de sua civilização milenar.

Por isso, se for o caso de especular sobre o futuro desta “nova era” que está nascendo, é preciso ter claro que a China não está se propondo a substituir os Estados Unidos como centro articulador de algum tipo de novo “projeto ético universal”. Tudo indica que o avanço desta nova “era multicivilizacional” já não tem como ser revertido, nem há mais como devolver o sistema mundial à sua situação anterior, de completa supremacia eurocêntrica. “E mesmo que o eixo do sistema mundial ainda não tenha se deslocado inteiramente para a Ásia, o certo é que já se estabeleceu um novo “balanço de poder” que deslocou a hegemonia anterior, do projeto universal e do “expansionismo catequético” da tradição greco-romana e judaico-cristã”[6].

Last month, I introduced you to Thingyan festival. This month I’ll tell you a little more about Thingyan festival, but from a different perspective, food, and for a couple of good reasons: 1) It’s April so that means it’s Thingyan season, and; 2) Everyone likes food and it’s a must to try the local food when you travel somewhere, right?

Like many other festivals around the world, Thingyan has its own unique festival food. As a Myanmar (or Burmese) myself, I feel like something is missing if I haven&#;t eaten Thingyan food during the season. It’s definitely a must for me. So, let me introduce you to the different iconic festival food I encountered as I grew up.

Before I start, I need to explain the word jaggery, because I will use it a lot. Jaggery, as I was taught to call it in English, is a sweet rock-sugar like solid extracted from a sugar palm tree that is used in many different desserts in Myanmar. It is the essential sweetener for most traditional Myanmar snacks. The liquid from the juice of the sugar palm tree is extracted and boiled down to a caramel-like consistency, and then cooled down to achieve the desired texture.

Shwe-yin-aye & Mont-lat-saung(assorted jelly, bread and sticky rice in coconut milk or watered down jaggery caramel sauce)

Shwe-yin-aye and Mont-lat-saung are favourite Thingyan desserts/sweet snacks, and although they are sometimes grouped together, they are not the same. Here is a handy guide on how to distinguish them. Mont-lat-saung only has one type of jelly, and no bread or sticky rice. Depending on the area, you might get different colours of jelly – either green or white  &#; and the sauces will be different too. From what my mother has told me, the people from Southern Myanmar usually eat the dish with white jelly and jaggery caramel sauce, while the people from the area around Yangon and Mandalay enjoy eating green jelly with icy coconut milk. Even though I&#;m calling it jelly, it’s not actually made from agar like regular jelly. It’s actually just little bits of sticky rice, pounded and squeezed to get the particular shape we’re after. Technically, it is called mont-phat (literally meaning dough pieces)but we’ll call it jelly here to avoid confusion.

Mont-lat-saung is the crème de la crème for me during the festival season because it is very cooling and sweet – perfect for the weather. It is always served with ice-cold coconut milk over white bread, sticky rice and different colours of assorted jellies. The jellies are all different shapes – strings, neatly cut squares, little spheres, anything you like. Unlike Shwe-yin-aye, there are no variations to Mont-lat-saung.

Mont-lone-yay-paw (sticky rice balls with jaggery filled centres topped with freshly grated coconut)

Mont-lone-yay-paw (pictured at the top) is one of, if not the,most popular and iconic foods that represent the festival. Almost everyone makes and eats this during the festival. From what I have learned throughout the years, it is considered Thingyan food because of the way it’s made. First of all, you have to mix rice flour and sticky rice flour into a dough with a particular texture. Then you break off little pieces of the dough, add jaggery to the centre and then roll the pieces into little balls, making sure that the sweet centre is sealed. The dough balls are boiled for about minute in a pot of water. When the dough balls are thoroughly cooked, they float to the surface. The cooked pieces are then washed in cold water before being served.

Due to the tedious nature of the cooking process, a lot of people come together to prepare mont-lone-yay-paw. And since the festival revolves around family and friends being together, it is a perfect family food, as everyone can bond while making these treats and eating them.

Of course, Thingyan is also the time when cheeky pranks are played, and this dish doesn&#;t get the exemption stamp. Instead of adding jaggery, mischievous souls like to put hot fresh chillies inside to play a prank on others. And you just cannot get mad at anyone who plays a prank as it is a Thingyan tradition that has been practised for many, many years. So, if you are going to a friend’s house, make sure your senses are on red alert if you are served mont-lone-yay-paw, just in case you&#;re the one who gets attacked by this chilli bomb.

Thingyan hta-minn (literal translation – Thingyan rice)

To those who think porridges are disgusting and should be abandoned from planet earth, you are advised to stop reading because this as it is going to sound extremely bizarre. This dish is essentially cooked rice in scented water.

What?!

Yes, it is cooked rice in scented water. It is a legitimate food in the Mon state where my mom grew up. Rice is cooked and then washed to get a fluffy and light texture. Sandalwood is burnt with a wax candle, and is then infused with ice-cold water, to give it a fragrant smell. This sandalwood water is then poured on top of the rice, and this rice with scented water is eaten with various side dishes, such as dried fish, fried chillies, assorted pickled salads and other savoury dishes. My favourite side dish is the pickled mango salad, which has the perfect balance of saltiness, spiciness and tanginess.

I still remember visiting my late grandmother’s house during the festival and eating Thingyan hta-minn. You eat the plain rice, which is cold but pleasingly fragrant, and you follow it up with the side dishes, which are usually slightly spicy and salty. It is definitely a dish that you should try if you are ever around Mon state during the festival. It might sound strange, but I highly recommend that you bite the bullet and try it. It’ll either be a hit or a miss, like marmite!

I really enjoy eating these three dishes when I’m home around April, Thingyan festival season, in Myanmar. It is going to be water festival soon and my blood is tingling with the longing to participate in the festival, but I have a pile of assignments to plow through…

Vanaf het midden van de jaren twintig trok hij regelmatig naar Parijs, waar hij een groot aantal doeken met gezichten op de brede boulevards heeft geschilderd. Tot op hoge leeftijd heeft hij doorgewerkt en een oeuvre opgebouwd dat uit ontelbare schilderijen, aquarellen en tekeningen bestaat van de meest uiteenlopende onderwerpen: landschappen, stads- en dorpsgezichten, kermis en circustaferelen, het boerenleven, portretten enz. Veel succes heeft hij geoogst met zijn schilderijen en aquarellen van naakte kinderen die in de Drentse Aa zwemmen en spelen, een thema dat hij sinds de jaren dertig veelvuldig heeft vastgelegd.
